At Nagoya University, a Harassment Prevention Committee has been established for the purpose of setting up a system for harassment prevention, receiving redress requests, etc. The Committee seeks to implement appropriate response measures, through overseeing the system for the University’s response to harassment and pursuing cooperative relationships with related departments and specialized institutes. In addition, a Harassment Consultation Center has been founded to appropriately conduct consultations and respond to various forms of harassment and similar violations of human rights, as well as issues arising from them. The Harassment Consultation Center offers consultations and carries out activities to prevent harassment.

　In this questionnaire, we ask you about your awareness of harassment as well as your educational, research, work, and academic environment with the aim of enriching Nagoya University’s harassment prevention system. The results of this questionnaire will be used for the activities of the Harassment Consultation Center, and a portion of the results will be published on the website of the Center for on-campus access only, but all answers will be processed and analyzed statistically and will never be published in a form by which individuals may be identified.

　In the questionnaire, there are questions about the experiences of the respondents themselves, as well as questions about experiences the respondents have witnessed. For this questionnaire, please answer with any experiences that have occurred with Nagoya University members over the previous year. If you enrolled in Nagoya University in AY 2022 onwards, please answer with any experiences that have occurred with Nagoya University members during your time since enrollment.
